What should you know about asymptomatic pneumonia?

It may be found incidentally on chest imaging or during an assessment for another concern. A clinician interprets the image alongside your examination, medical history and, when appropriate, further tests because an image alone does not always establish the cause.

Some people have no cough, fever, breathlessness or chest discomfort when an abnormal area first appears in the lung. Others have subtle changes such as unusual tiredness, reduced appetite or lower exercise tolerance that they did not connect with a respiratory illness.

The finding can have infectious and non-infectious explanations. Viral illnesses, including influenza and COVID-19, may involve the lungs, while old scarring or another inflammatory process can resemble infection on an image. Clinical assessment helps distinguish these possibilities.

Arrange follow-up with the clinician who ordered or reviewed the imaging. Bring the report, a medication list and details of recent infections, travel, swallowing problems, immune conditions and exposure to anyone with a respiratory illness.

Do not start leftover antibiotics. Treatment depends on the suspected cause, the area involved and your overall health. Follow any plan for repeat examination, testing or imaging, and watch for new cough, phlegm, fever, chills, chest pain or shortness of breath.

Seek urgent medical help for new or worsening difficulty breathing, blue or grey lips, severe chest pain, confusion, fainting, marked drowsiness, or inability to keep fluids down. These warning signs matter even if the original lung finding caused no symptoms.

Contact a clinician promptly if fever develops, a cough becomes persistent, phlegm is blood-stained, or your usual activity suddenly feels much harder. People with weakened immunity, significant heart or lung disease, or frailty should have a lower threshold for assessment.

Ask what the image actually showed, whether previous images are available for comparison, what causes are being considered, and whether you need blood tests, a respiratory sample or repeat imaging. Clarify which symptoms should change the timing of your follow-up and whether you should limit close contact with others while infection is being assessed.
