The short answer
Croup involves swelling around the upper airway and is seen mainly in young children, often during a viral illness. Asthma affects the smaller breathing tubes in the lungs. A child can have asthma and develop croup, so the sound and effort of breathing matter.
Where the breathing noise comes from
Croup often produces a harsh, seal-like bark and stridor, a rough high-pitched sound most noticeable while breathing in. Hoarseness may occur. Asthma wheeze is usually a musical sound from the chest, often clearer while breathing out, with breathlessness or tightness.
Families may find breathing sounds hard to identify, especially when a child is upset. A recording made without delaying care can help a clinician, but breathing effort, colour, alertness, and ability to drink are more important than naming the sound correctly.
Keep the child calm and observe breathing
Hold or sit the child upright in a comfortable position and offer fluids if swallowing is easy. Distress and crying can make upper-airway noise louder. Avoid steam because it can burn and does not replace medical assessment.
If the child has an asthma action plan and the symptoms fit their usual asthma pattern, follow it. An asthma inhaler does not treat upper-airway swelling from croup. Contact a clinician for a barking cough with noisy breathing so the need for croup-specific treatment can be assessed.
Signs a child needs urgent help
Seek emergency care for stridor while the child is resting, severe effort to breathe, blue or grey lips, drooling or inability to swallow, pauses in breathing, unusual sleepiness, confusion, or a child who is becoming exhausted.
Do not inspect the throat or force a distressed child to lie down when breathing is difficult. Call local emergency services if symptoms are severe or deteriorating, whether the suspected cause is croup, asthma, or another airway problem.
