Antibiotics for a lung infection

Short answer
Antibiotics treat bacterial lung infections, but they do not work against viral causes such as influenza or COVID-19.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Cause
- Bacterial and viral lung infections require different treatment decisions.
- Assessment
- Breathing, oxygenation, circulation, hydration, and alertness help show severity.
- Recheck
- Worsening or absent improvement should prompt reassessment.
The short answer
The decision depends on symptoms, examination findings, illness severity, medical history, and sometimes imaging or samples. A cough or phlegm colour cannot select an antibiotic by itself.
Identifying the cause and severity
A lung infection may involve the airways or the air sacs. Pneumonia can present with cough, fever, breathlessness, chest discomfort, chills, or confusion, but the pattern varies. Viral infections can produce similar symptoms and may occur alongside a bacterial infection.
Clinicians assess breathing effort, oxygen level, circulation, hydration, and alertness. The person's age, pregnancy status, immune health, long-term conditions, medicine allergies, and recent antibiotic use can influence testing and treatment.
Following the treatment plan
If antibiotics are prescribed, take them on schedule and follow food or storage instructions. Ask a pharmacist what to do about a missed dose. Never use a leftover course, because the cause, dose, and required medicine may differ.
Rest, drink enough fluid unless you have a fluid restriction, and avoid smoke exposure. Monitor whether breathing, alertness, fluid intake, and overall strength are improving. Seek reassessment if symptoms worsen or fail to improve as your clinician expected; the diagnosis or treatment may need to change.
Breathing and severe-infection warning signs
Call emergency services for severe difficulty breathing, blue or grey lips, collapse, new confusion, inability to stay awake, or rapidly worsening illness. Do not drive yourself if you are faint, confused, or struggling to breathe.
Urgent assessment is also needed for coughing up blood, severe chest pain, very little urine, inability to keep fluids down, or a marked deterioration in someone who is pregnant, immunocompromised, frail, or living with serious heart or lung disease. These symptoms drive urgency; they do not reveal the organism.
Treatment decisions to discuss
Ask whether the working diagnosis is pneumonia or another respiratory illness, what suggests a bacterial cause, and whether any test result is pending. Confirm when improvement is expected, how follow-up will occur, and exactly which change in breathing or alertness should prompt urgent care.

Questions people ask
Does coloured phlegm prove I need antibiotics?
No. Phlegm colour can change during different respiratory illnesses and cannot confirm a bacterial cause on its own.
Can antibiotics treat influenza or COVID-19?
Antibiotics do not treat these viruses. They may be prescribed only if a clinician also identifies or strongly suspects a bacterial infection.
Why might a clinician request a chest image or sample?
These checks can help assess the location and extent of infection or identify an organism. Whether they are useful depends on illness severity and the clinical findings.
