The short answer
Difficulty conceiving can also be a presentation of recurrent anovulation. Symptoms and cycle calendars provide clues, but they cannot confirm egg release or identify the underlying cause on their own.
Patterns you may notice
When an egg is not released, the expected hormone sequence changes. Periods may come far apart, disappear, or arrive with a variable flow. Bleeding can be light or heavy because the uterine lining develops and sheds unpredictably.
Other features depend on the cause. Polycystic ovary syndrome can present with irregular periods alongside acne or increased facial and body hair. Premature ovarian insufficiency can present with missing periods and symptoms such as hot flushes or vaginal dryness. These combinations support an assessment; they do not establish a diagnosis.
What to record and check
Log the first and last day of bleeding, changes in flow, pelvic pain, skin or hair changes, temperature symptoms, medicines, contraception and attempts to conceive. Test for pregnancy whenever it is possible and a period is late, regardless of whether your cycles are usually irregular.
A clinician may consider your health history, examination, blood tests or pelvic ultrasound. Tests are chosen to look for pregnancy and possible hormonal or structural causes rather than to label the bleeding pattern alone.
Signs that need medical care
Arrange a non-urgent review for repeatedly missed or markedly irregular periods, new excess facial hair, persistent milky nipple discharge when not breastfeeding, or difficulty conceiving. These features can guide which hormone checks are useful.
Seek urgent help for severe pelvic pain, collapse, faintness or very heavy bleeding, especially when pregnancy is possible. A positive pregnancy test with pain or bleeding needs prompt assessment.
