Wheezing is not always asthma

Asthma often presents with variable wheeze, cough, breathlessness or chest tightness, especially after exercise, at night or around triggers. Infections, allergic reactions, inhaled objects, smoking-related lung disease, vocal-cord problems and heart conditions can produce similar sounds or sensations.

True wheeze is usually musical and more noticeable when breathing out. A harsh sound mainly while breathing in may come from the upper airway and needs a different assessment. Sudden wheezing after food, medicine or an insect sting raises concern for an allergic reaction; sudden onset while eating or playing can suggest an inhaled object.

Asthma cannot be confirmed by sound alone. A clinician reviews when symptoms occur, trigger patterns, smoking or workplace exposures, previous allergies and response to treatment. Examination and breathing tests may be needed, sometimes repeated because asthma-related narrowing can vary over time.

Sit upright, move away from smoke or fumes and keep activity gentle while assessing your breathing. If you have a prescribed reliever inhaler and a personal action plan, use them exactly as directed. Do not borrow another person's inhaler or assume that every episode needs the same treatment.

New, recurrent or unexplained wheezing warrants a clinical review. Record whether it follows exercise, infection, dust, pets, meals or medicine, and whether cough or chest tightness occurs with it. Nebuliser treatment delivers medicine as a mist, but the need for it depends on severity and diagnosis.

Call emergency services if the person struggles to speak, becomes drowsy or confused, develops blue or grey lips, has severe chest pulling, collapses, or rapidly worsens despite prescribed rescue medicine. Wheezing with swelling of the lips or tongue, widespread hives or faintness after a possible allergen is also an emergency.

A choking episode with continued cough, noisy breathing or one-sided wheeze needs urgent evaluation even if the person can still breathe. Infants, older adults and people with heart or lung disease should be assessed promptly when wheezing is new or accompanied by feeding difficulty, dehydration or breathlessness at rest.

Ask whether the sound is wheeze or upper-airway noise, which breathing tests are appropriate, and how to use any inhaler with correct technique. Confirm what should trigger your action plan, when treatment response should be reviewed and whether allergy or workplace exposures need investigation.
