When an ectopic pregnancy can be detected

Pregnancy timing is usually counted from the first day of the last menstrual period, not from conception. A pregnancy may be too early to locate on an initial scan. Around five to six weeks from the last menstrual period, a transvaginal ultrasound may be able to show where the pregnancy is developing, but cycle dates, hormone levels and scan findings all affect interpretation.

A positive pregnancy test confirms pregnancy hormone is present, but it does not identify where the pregnancy is located. Clinicians may measure hCG in the blood and repeat it after an interval to assess its pattern. They interpret that pattern alongside a transvaginal ultrasound, symptoms and examination rather than using one value as a diagnosis.

When a scan does not yet show a pregnancy inside or outside the uterus, clinicians may call it a pregnancy of unknown location. This is a temporary classification that needs follow-up until the location or outcome becomes clear. Ectopic pregnancy can present with one-sided pelvic pain, vaginal bleeding, shoulder-tip pain, dizziness or fainting, although some people have few symptoms early on.

Contact a maternity service, gynaecology service or emergency department if you have a positive pregnancy test with pelvic pain or bleeding. Tell them the date of your last menstrual period, test results, fertility treatment, previous ectopic pregnancy, tubal surgery or pelvic infection. These details help determine the safest assessment plan.

Attend every requested blood test and scan until the pregnancy location is confirmed or your clinical team says monitoring is complete. Keep clear instructions for whom to contact if symptoms change. Do not rely on improving pain, a single hormone result or one inconclusive scan to end follow-up yourself.

Seek emergency care now for sudden or severe abdominal or pelvic pain, shoulder-tip pain, fainting, marked dizziness, pale or clammy skin, confusion, breathing difficulty, or heavy bleeding with weakness. These can be signs of internal bleeding or a ruptured ectopic pregnancy. Urgency is based on the symptoms, even if the pregnancy test is faint, the hCG level is low or an earlier scan was inconclusive.

Ask what the scan did and did not show, when hCG will be repeated, and what changes would trigger another scan or treatment. If ectopic pregnancy is confirmed, discuss whether monitoring, medicine or surgery fits your condition, what each option involves, and where to go if pain or bleeding worsens outside clinic hours.
