Adenomyosis ultrasound features

The examiner looks for a combination of signs and interprets them alongside heavy or painful periods, pelvic pain, examination findings and other possible gynaecological conditions.

Possible signs include uneven thickening of the womb muscle, a rounded or enlarged womb, tiny cyst-like spaces within the muscle, fan-shaped shadowing, bright islands or lines, and an irregular boundary between the womb lining and muscle.

A report may call the pattern focal when it is concentrated in one area or diffuse when changes are spread through the muscle. Some features overlap with fibroids, prior surgery and normal variation, so the overall pattern matters more than an isolated phrase.

The scan appearance does not reliably measure symptom burden. Mild-looking changes may accompany substantial pain or bleeding, while a conspicuous pattern may cause little disruption.

Ultrasound may be performed over the lower abdomen or with an internal probe. Follow the clinic's bladder instructions, mention pregnancy possibility and explain if an internal scan would be difficult. The procedure requires consent, and you may ask to pause at any point.

Review the report with the clinician who knows your symptoms. Ask whether the findings adequately explain the bleeding or pain, whether fibroids or endometriosis are also suspected, and whether MRI would add information that changes management. Treatment is chosen around symptoms and fertility preferences rather than scan wording alone.

Seek urgent help for sudden intense pelvic pain, fainting, severe dizziness, chest discomfort, breathlessness, or very heavy bleeding with weakness. Pain or bleeding alongside a positive pregnancy test also requires urgent assessment, regardless of a previous ultrasound report.

Request a timely review for worsening period pain, increasingly heavy bleeding, spotting between periods, pain during sex or fatigue that interferes with daily activities. A scan that does not show clear adenomyosis should not end the assessment of persistent symptoms.

Consider asking which specific features were present, whether they were focal or diffuse, and how confident the interpretation is. Find out whether another condition was visible, whether further imaging would alter care, and which options suit your pregnancy plans.
