Does adenomyosis have stages?

A report may describe adenomyosis as focal or diffuse, or comment on how much of the womb muscle appears involved. Those descriptions help communicate what imaging shows, but they are not equivalent to a standard sequence of disease stages.

Focal adenomyosis affects a more localised part of the womb muscle, while diffuse adenomyosis is spread more broadly through it. Imaging may also describe an enlarged or uneven womb, small cyst-like spaces in the muscle, or an indistinct boundary between the lining and muscle.

The apparent extent on a scan does not neatly predict how someone feels. A person with subtle imaging changes may have disruptive pain or bleeding, while more extensive-looking changes may cause fewer symptoms. Severity is therefore discussed through lived impact as well as scan findings.

Track bleeding, period pain, pelvic pressure, pain during sex and effects on sleep or daily life. Tell your clinician whether you are trying to conceive or want to preserve the option of pregnancy, because this changes which treatments are suitable.

Management may range from pain relief and hormonal options to procedures or surgery. The choice depends on the main symptom, examination and imaging findings, other gynaecological conditions, previous treatment, fertility goals and your preferences—not a stage label alone.

Arrange a review when heavy or painful periods are worsening, pelvic pain persists beyond menstruation, bleeding occurs between periods, or fatigue and breathlessness suggest possible anaemia. A changing pattern may justify reassessment even after adenomyosis has been diagnosed.

Seek urgent help for sudden severe pelvic pain, fainting, severe dizziness, chest discomfort, breathlessness at rest or very heavy bleeding with weakness. Urgency comes from these warning signs rather than any description of imaging extent.

Ask whether the findings are focal or diffuse, whether fibroids or endometriosis may also contribute, and how well the scan matches your symptoms. Clarify what the suggested treatment aims to improve and how it fits your plans for pregnancy.
