Adenomyosis pain without a period

Adenomyosis involves tissue similar to the uterine lining growing within the muscular wall of the uterus. The affected muscle may feel tender or cramp even outside menstrual bleeding. Pain can be constant, come in waves, or worsen around ovulation, sex or physical activity.

Pain between periods can fit with adenomyosis, particularly in someone already diagnosed, but it is not specific to this condition. Ovarian cysts, endometriosis, fibroids, pelvic infection, urinary problems and bowel conditions can cause overlapping discomfort. Pregnancy-related causes also need consideration when pregnancy is possible.

The location and pattern offer useful clues. Note whether the pain is central or one-sided, whether it follows a cycle, and whether bleeding, discharge, fever, nausea, bowel changes, urinary symptoms or pain during sex occur with it.

Keep a symptom diary across your cycle, including bleeding, pain location, triggers and the effect on sleep or daily activities. A warm pack and gentle movement may ease cramping. If you use a non-prescription pain reliever, check that it is suitable for you, especially if you have stomach, kidney or bleeding problems, take other medicines, or could be pregnant.

Arrange a gynaecology review if pain continues, changes from your usual pattern or is affecting daily life. The clinician may discuss an examination, pregnancy testing when relevant, or imaging to look for adenomyosis and other causes.

Seek urgent medical help for sudden severe pelvic pain, fainting, marked dizziness, shoulder-tip pain, heavy bleeding, fever, repeated vomiting or a rigid or very tender abdomen. Get prompt assessment for pelvic pain with a positive pregnancy test or a missed period, because causes outside adenomyosis may require time-sensitive care.

Ask what else could explain pain outside bleeding, whether imaging would change management, and which pain relief is compatible with your health and medicines. If symptoms are recurring, discuss whether hormonal treatment or a procedure matches your priorities, including contraception, future pregnancy and preserving the uterus.
