Adenomyosis pain after a period

Pain may feel cramping, aching, pressure or tenderness and can extend beyond the days of visible bleeding. A change from your usual pattern deserves review, particularly when pain becomes more intense, occurs between cycles or is accompanied by bowel, bladder or sexual symptoms.

In adenomyosis, tissue within the uterine muscle responds to hormonal cycling. The uterus may remain tender and the muscle may continue to contract after the main bleeding has finished. Pelvic floor muscles can also tighten in response to recurring pain, adding soreness or discomfort with movement.

Timing alone does not confirm adenomyosis as the cause. Endometriosis, fibroids, ovarian problems, infection and bowel or bladder conditions can overlap with pelvic pain. A clinician uses the full symptom pattern, examination when appropriate and imaging to decide what needs investigating.

Use a cycle diary to mark when bleeding starts and stops, where pain is felt, what it feels like, and whether it changes with sex, urination, bowel movements or activity. Note medicines taken and whether they helped. This record can distinguish pain tied closely to menstruation from discomfort present through much of the month.

Heat and gentle movement may ease some cramps. Use pain medicine only as directed and check with a pharmacist or clinician if you have stomach, kidney or bleeding problems, take other medicines, or could be pregnant. Persistent pain may require a review of hormonal treatment or other options.

Get urgent assessment for sudden severe pain, fainting, marked dizziness, fever, repeated vomiting, a rigid or very tender abdomen, or pain with possible pregnancy. These features can signal a problem other than a routine adenomyosis flare.

Schedule a gynaecology review when post-period pain is new, steadily worsening, disrupting sleep or daily life, or continuing despite your current plan. Report unusual discharge, bleeding between periods, pain during sex or new bowel and bladder symptoms.
