Adenomyosis when you have no period

Pregnancy, menopause, hormonal contraception and changes in ovulation can all alter or stop bleeding. Adenomyosis describes tissue within the muscle of the uterus; it does not require a period to remain present, although its cycle-related symptoms may change.

A hormonal coil, injection, pill or other hormonal treatment may make bleeding light or absent while still controlling adenomyosis symptoms. After menopause, bleeding should have stopped, but pelvic pressure or pain can still merit review rather than being assumed to come from adenomyosis.

If periods stop unexpectedly, clinicians consider pregnancy first when it is possible. They may also assess medication effects, stress, weight or exercise changes, thyroid or other hormonal problems, and the transition toward menopause. Pelvic pain alone cannot identify which explanation applies.

Take a home pregnancy test if pregnancy is possible and follow its instructions. Note the date of your last bleed, contraception, pain pattern, discharge, medicines and menopausal symptoms. Arrange a clinical review if the change is unexplained, persists, or comes with new pelvic symptoms.

Do not discontinue a prescribed hormonal treatment solely because bleeding has stopped. Ask whether absence of bleeding is an expected effect, whether contraception is still needed, and whether examination, blood tests or imaging would help clarify the cause.

Seek urgent help for severe or one-sided pelvic pain with a missed period, especially with dizziness, fainting, shoulder-tip pain or bleeding, because an ectopic pregnancy must be excluded. Fever, collapse, repeated vomiting or rapidly worsening pain also warrants prompt assessment.

Any bleeding after menopause should be medically assessed. Contact a clinician soon if a pregnancy test is positive and you have pain, or if pelvic discomfort continues despite treatment.
