New treatment options for adenomyosis

There is no single new treatment that suits everyone. Options may include hormonal treatment, medicines for pain or bleeding, uterus-sparing procedures and hysterectomy. The right choice depends on symptoms, scan findings, other pelvic conditions and whether pregnancy is desired.

Hormonal approaches aim to reduce bleeding and cycle-related pain while treatment continues. A hormonal coil acts mainly inside the uterus and may be considered when the uterine cavity can accommodate it. Uterine artery embolisation reduces blood flow to affected uterine tissue; suitability requires specialist assessment.

Endometrial ablation treats the uterine lining, not adenomyosis within the muscle, so it is not suitable for every pattern of disease and is not used when future pregnancy is planned. Hysterectomy removes the uterus and is the definitive surgical option, but it ends the ability to carry a pregnancy.

Record when bleeding and pain occur, how they affect sleep or daily activity, and which treatments you have already tried. Bring details of medicines, previous scans, pregnancy plans and any preference to preserve the uterus. A gynaecologist may review the diagnosis and check whether fibroids or endometriosis could also be contributing.

Ask what the treatment is intended to improve, how soon benefit may be noticed, what side effects or recovery to expect, and what happens if symptoms continue. Do not stop prescribed hormonal medicine without discussing an alternative and contraception needs.

Arrange urgent medical assessment for very heavy bleeding with faintness, breathlessness, chest discomfort or marked weakness. Sudden severe pelvic pain, fever, repeated vomiting, collapse, or pain with a possible pregnancy also needs prompt evaluation because another condition may require immediate treatment.

Book a routine review when pain or bleeding is worsening, treatment side effects are troublesome, or symptoms interfere with work, sleep, exercise or sex.
