Clinical features of adenomyosis

Symptoms can cluster around menstruation or extend through the cycle. On examination, a clinician may find that the uterus feels enlarged, rounded, or tender, although an examination can also be normal. Clinical features describe the pattern; they do not confirm the diagnosis alone.

Heavy bleeding can include flooding, clots, night-time changes, or periods that disrupt usual activities. Pain may feel cramping, aching, heaviness, or pressure in the lower abdomen and pelvis. Ongoing blood loss can lead to tiredness, dizziness, paleness, headaches, or breathlessness from anaemia.

Fibroids, endometriosis, pelvic infection, and other causes of abnormal bleeding can produce overlapping features. Adenomyosis often presents with heavy or painful periods, but having either symptom does not show which condition is responsible.

Track bleeding, clots, pain location, pain timing, pain during sex, and medicines taken across several cycles if practical. Record whether you miss work, wake at night, avoid activity, or need to change menstrual protection unexpectedly.

The clinician may discuss a pelvic examination, blood tests for anaemia, and imaging such as ultrasound. Treatment is tailored to the dominant symptoms, other health conditions, contraceptive needs, and plans for pregnancy.

Obtain urgent help if heavy bleeding comes with fainting, severe dizziness, shortness of breath, confusion, chest discomfort, or extreme weakness. Seek rapid assessment for sudden intense pelvic pain, fever, repeated vomiting, or pelvic pain with a possible pregnancy. Bleeding after menopause should be medically assessed even when it is light.

Ask which features point toward adenomyosis, which other causes need consideration, and whether you should be checked for anaemia. Discuss how treatment choices address bleeding versus pain and whether they affect contraception, fertility, or the timing of pregnancy.
