Birth control for adenomyosis

Adenomyosis commonly presents with heavy or painful periods because tissue within the womb muscle responds to menstrual hormones. Hormonal contraception can thin the womb lining, suppress ovulation or reduce menstrual cycling, depending on the method. This may make bleeding lighter and cramps easier to manage, although the response and side effects differ between individuals.

Options may include a hormonal coil, progestogen tablets, an implant, an injection, or a combined pill, patch or ring. Not every method is suitable for every person. Migraine pattern, blood-clot history, smoking, blood pressure, breastfeeding, other medicines and preference about bleeding all influence the choice.

Explain whether your main aim is contraception, symptom control or both. Tell the clinician if you hope to conceive soon, have previously had troublesome side effects, or take medicines that could alter contraceptive effectiveness. Ask what bleeding changes are expected after starting and when the method will provide contraceptive protection.

Use the method exactly as directed and arrange follow-up if bleeding or pain remains disruptive. Irregular bleeding can occur after a hormonal method is started, but persistent symptoms may need review for placement problems, anaemia, pregnancy or another gynaecological cause. Birth control manages symptoms; it does not confirm the diagnosis.

Contact your clinician if side effects are difficult to tolerate, bleeding becomes heavier, pain worsens, or you cannot feel coil threads after previously being able to feel them. Seek urgent care for chest pain, sudden breathlessness, coughing blood, one-sided leg swelling, fainting, sudden severe pelvic pain, or possible pregnancy with pain or bleeding.

Ask which methods are safe with your medical history, how each is expected to change bleeding, and which side effects merit an earlier review. Confirm what to do after a missed pill, late injection, expelled coil or medicine interaction, and discuss non-contraceptive alternatives if pregnancy is currently desired.
