Adenomyosis during and after menopause

During perimenopause, periods can become irregular while adenomyosis continues to cause heavy bleeding, cramps, pressure, or pelvic discomfort. Once menstrual cycles have stopped, falling ovarian hormone levels may reduce adenomyosis activity, though symptom improvement is not identical for everyone.

Irregular perimenopausal bleeding can overlap with adenomyosis, fibroids, polyps, changes in the womb lining, medicine effects, and other gynaecological causes. A previous adenomyosis diagnosis does not explain every later episode of bleeding.

The clinician may consider your bleeding pattern, pain, medicines, menopausal hormone therapy, and personal health history. Depending on the symptoms, evaluation may include examination, ultrasound, blood tests, or a closer assessment of the womb lining.

Write down bleeding dates, clots, pain, pressure, and effects on sleep or activity. Record when menopause-related or hormonal medicines began, since this helps the clinician compare symptoms with treatment changes.

Before menopause, options may include pain relief, medicines that reduce bleeding, hormonal treatment, or procedures. Decisions should account for symptom severity, other health conditions, contraception needs, pregnancy wishes, and how near natural menopause may be. After menopause, treatment focuses first on identifying the cause of new symptoms.

Contact a healthcare professional promptly for any vaginal bleeding after menopause, including light spotting or a blood-stained discharge. This advice applies even when adenomyosis was diagnosed before periods stopped or when hormonal medicine is being used.

Seek urgent care if bleeding is heavy and accompanied by fainting, marked dizziness, breathlessness, chest discomfort, or severe weakness. Sudden intense pelvic or abdominal pain, fever with feeling acutely unwell, or collapse also warrants urgent assessment.

Ask whether your current symptoms fit adenomyosis or need investigation for another cause, and whether any medicine could be contributing. If menopausal hormone therapy is being considered, discuss how your bleeding history affects the choice and what changes should trigger a review.
