Acids for Whiteheads: What to Know

Whiteheads are closed comedones: oil and dead skin collect beneath a pore opening that remains covered. A leave-on salicylic acid product can enter oily pores, while alpha-hydroxy acids work more at the skin surface. Neither should be used as a reason to squeeze bumps.

Comedonal acne commonly presents with whiteheads and blackheads, often with little redness. Firm, pearl-like bumps that do not release pore material may instead be milia, which are small keratin-filled cysts and may not respond to acne acids in the same way.

Occlusive cosmetics, oily hair products, friction, and an irritating skin-care routine can contribute to clogged or inflamed skin. The location and appearance of the bumps help a clinician separate acne from folliculitis, milia, or irritation around the mouth.

Start with a low-strength, clearly labelled salicylic acid product on intact skin and follow its directions. Use it on fewer days initially, then adjust according to dryness and comfort. Keep the rest of the routine gentle: cleanse without scrubbing, moisturise with a non-comedogenic product, and wear sunscreen during the day.

Avoid picking, needles, harsh brushes, and repeated extractions at home because they can cause inflammation, infection, and dark marks. Do not begin several acne actives together; spacing out new products makes it easier to identify what causes stinging or a rash. Pregnancy or plans for pregnancy should be mentioned before choosing acne medicines with a clinician.

Stop using the acid and seek advice if you develop intense burning, facial swelling, blisters, crusting, or a widespread itchy rash. Urgent help is appropriate if swelling affects the lips or tongue or if breathing becomes difficult.

Arrange a dermatology appointment when bumps are painful, leave scars or persistent dark marks, spread despite consistent gentle care, or are difficult to identify. A clinician may recommend a topical retinoid for comedonal acne or perform sterile extraction for selected lesions; these approaches require care to reduce injury and pigment change.

Ask whether the bumps are closed comedones or milia, how frequently your chosen acid should be used, and which signs mean your skin barrier is becoming irritated. If whiteheads continue, ask whether a retinoid, prescription acne plan, or clinician-performed extraction would address the blockage more appropriately.
