What to Know About the ACDF Procedure

The surgeon reaches the cervical spine through the front of the neck. Whether it is appropriate depends on the location of compression, the symptoms, imaging findings, general health, and whether nonsurgical care has helped.

After the affected disc is removed, the surgeon clears tissue or bone that is pressing on nearby neural structures. A spacer or bone graft is placed in the disc space to preserve alignment and support fusion. A plate and screws may be added to hold the segment steady while the bones unite.

Fusion stops movement at the treated level. Nearby levels still move, although neck flexibility may feel different, especially when more than one level is treated. Arm pain from a compressed nerve may ease differently from numbness or weakness, which can recover more gradually.

Before surgery, give the care team a complete list of medicines and supplements, discuss smoking or nicotine use, and follow instructions about eating, drinking, and medicine changes. Arrange transport home and practical help for the early recovery period.

Afterwards, follow the wound-care, lifting, activity, and collar instructions supplied by the surgical team. Walking is commonly encouraged in short, comfortable amounts. Do not drive until the surgeon says it is safe and you can turn your head adequately without sedating medicine. Attend follow-up visits so healing, nerve function, and spinal alignment can be checked.

Seek emergency help for new trouble breathing, rapidly increasing neck swelling, severe difficulty swallowing, chest pain, fainting, or sudden weakness in an arm or leg. These symptoms require prompt assessment rather than waiting for a routine appointment.

Contact the surgical team promptly about fever, worsening wound redness, drainage, pain that is escalating despite the prescribed plan, new numbness, loss of bladder or bowel control, or calf swelling and tenderness. Mild throat discomfort can occur after an anterior approach, but worsening swallowing problems should be reported.

Ask which spinal level is causing the symptoms, what the imaging shows, and why ACDF is being recommended over continued rehabilitation or another operation. Clarify which implant or graft is planned, whether a neck collar will be used, and which activities will be restricted.

It is also useful to discuss the expected effect on arm symptoms versus neck pain, personal risks related to your health and medicines, the follow-up schedule, and the signs that should trigger an urgent call.
