What to Know About Two-Level ACDF

ACDF stands for anterior cervical discectomy and fusion. The surgeon approaches through the front of the neck, clears disc material and other structures causing compression, then places graft material and often an implant to support fusion.

A spinal level is the space between a pair of vertebrae. Two-level surgery treats two such spaces, usually next to each other. Compared with a single-level operation, it spans a longer portion of the neck and leaves the treated vertebrae moving together as the graft heals.

The aim is to address compression that matches the examination and imaging. Arm pain, tingling, numbness, weakness, hand clumsiness, or walking changes may be relevant, depending on whether nerve roots or the spinal cord are affected. The operation is selected for the individual pattern rather than for an imaging label alone.

Before surgery, give the team a complete medication list and disclose smoking or nicotine use, swallowing problems, previous neck operations, and any new weakness. Follow the hospital's instructions about medicines, food, drink, bathing, and arrival.

Afterward, use prescribed pain relief as directed, walk in short comfortable bouts, and follow restrictions for lifting, driving, wound care, and neck movement. A collar is used only when the surgeon recommends it. Attend follow-up so the team can assess symptoms, the incision, implants, alignment, and the progress of fusion.

Get urgent medical help for trouble breathing, rapidly increasing neck swelling, inability to swallow liquids, new or worsening arm or leg weakness, loss of bladder or bowel control, or sudden difficulty walking. These symptoms need prompt assessment because they can signal airway compromise or pressure on neurological structures.

Contact the surgical team promptly for fever with an increasingly red or draining incision, pain that escalates despite prescribed treatment, persistent vomiting, or a new medication reaction. Mild throat soreness or temporary swallowing discomfort can occur, but worsening symptoms still deserve review.

Ask which exact levels are planned, what structure is compressed, and how the findings explain your symptoms. Clarify what graft and implants may be used, which symptoms the operation is intended to improve, how your health affects fusion, and what activity limits will apply at home and work.
