Ectopic pregnancy at four weeks: diagnosis and urgent symptoms

An ectopic pregnancy implants outside the uterus, most often in a fallopian tube, and cannot develop normally there. It can rupture and cause internal bleeding; follow-up must continue until clinicians have located the pregnancy or confirmed that it has resolved.

A positive pregnancy test with nothing visible inside or outside the uterus is sometimes described as a pregnancy of unknown location. This can represent a very early pregnancy in the uterus, a pregnancy that is no longer developing, or an ectopic pregnancy. Symptoms alone cannot reliably separate these possibilities.

The care team may compare pregnancy-hormone blood results over time and repeat a transvaginal ultrasound. Attend every arranged test even if pain or bleeding settles, because improvement in symptoms does not establish where the pregnancy is.

Contact an early-pregnancy or obstetric service promptly if you have a positive test with one-sided lower abdominal pain, vaginal bleeding, brown discharge, or shoulder-tip pain. Tell them the date of your last period, test results, symptoms, previous ectopic pregnancy, tubal surgery, fertility treatment, or history of pelvic infection.

Management depends on your symptoms, scan findings, hormone pattern, and overall health. Options can include close monitoring, medicine, or surgery. A clinician should explain why an option fits and how follow-up will show whether treatment has worked.

Call emergency services or go to an emergency department immediately for sudden severe abdominal pain, shoulder-tip pain with weakness, fainting, severe dizziness, pallor, confusion, or collapse. These can occur with internal bleeding from a ruptured ectopic pregnancy.

Do not wait for a scheduled scan or a heavier bleed. Internal bleeding may occur without heavy vaginal bleeding, and urgent action is based on the warning symptoms themselves.

Ask what is known about the pregnancy's location, when blood tests or ultrasound should be repeated, and exactly which symptoms should take you to emergency care. If treatment is recommended, discuss its effects, the required monitoring, and when it would be medically appropriate to try for pregnancy again.
