Two different breathing-related experiences

Cough is a reflex that clears or responds to airway irritation, whereas shortness of breath is the feeling that breathing takes more effort or does not provide enough air.

They can occur separately or together. Cough may feel dry, tickly or mucus-producing. Shortness of breath may be noticed at rest, with walking, when lying flat, or during a specific activity. Make cough the focus when it is the main change

Record when it started, whether anything is brought up, whether it is worse overnight, and possible triggers such as smoke, dust, cold air, meals or exercise. A cough can occur with respiratory infections, airway irritation, asthma-like symptoms and reflux; its cause needs the surrounding history and examination.

Rest, fluids and avoiding smoke may ease irritation. Do not rely on cough remedies to manage chest pain, wheeze or a worsening ability to breathe.

Make shortness of breath the priority when breathing feels limited

Treat breathlessness as the main concern when you need to pause while speaking or moving, feel unable to draw a satisfying breath, or notice that ordinary activity now requires more effort. Sit upright, loosen restrictive clothing and move away from smoke or fumes while arranging assessment. If you have a prescribed inhaler or breathing plan, follow the instructions already given for you.

Shortness of breath can accompany asthma, infections, heart problems, anxiety and other conditions. It should not be self-diagnosed from the sensation alone.

When to seek emergency help

Seek emergency care now for severe or sudden breathlessness, chest pain or pressure, fainting, confusion, blue or grey lips, coughing blood, or inability to speak in full sentences. These warning signs require urgent assessment even if a cough seems mild.

Prompt medical review is also appropriate for breathlessness that is new, keeps returning, worsens with less activity, or comes with swollen legs, fever, wheeze or a persistent cough. People with known heart or lung disease should follow their existing action plan and seek earlier help when symptoms depart from their usual pattern.

When another symptom deserves centre stage

Chest discomfort, palpitations, fever, sore throat, ankle swelling, panic symptoms or fatigue may provide the more useful starting point. Do not force the experience into either label if the main issue is pain, dizziness or a change in alertness. A short symptom timeline can help identify whether cough and breathlessness appeared together or one followed the other.
