What separates these symptoms
Coughing is a protective reflex that clears irritation, fluid, or material from the throat and airways. It may be dry, tickly, harsh, or intermittent. Coughing up phlegm, also called sputum, means that mucus comes into the mouth during a cough. The phlegm may be clear, white, yellow, green, brown, blood-streaked, thick, or thin; its appearance is only one part of the overall clinical picture.
A cough is a forceful clearing reflex, while coughing up phlegm means mucus is brought out from the airways.
Use cough when mucus is not coming out
This label fits a dry or tickling cough, or a cough where you do not see mucus. Irritation from a viral illness, smoke, dust, reflux, allergies, or medicines can trigger coughing. Notice whether it is worse at night, after activity, around particular exposures, or when lying down. Avoid smoke and other airway irritants, sip fluids if safe for you, and seek advice if the cough persists or interferes with sleep or normal activity.
Use coughing up phlegm when mucus is present
Use this description if material comes from the chest or throat when you cough. Mucus production can increase during respiratory infections, airway irritation, allergies, or long-term lung conditions. Rather than trying to identify a cause from colour alone, track whether the amount is increasing, whether it has blood in it, and whether there is fever, wheeze, breathlessness, chest discomfort, or a change from your usual pattern.
Warning signs that need prompt care
Get urgent medical help for severe difficulty breathing, blue or grey lips, severe chest pain, confusion, fainting, or coughing up more than a small streak of blood. Seek prompt assessment for blood-stained phlegm, new wheezing, high fever with feeling very unwell, dehydration, or a cough that is getting worse rather than easing. People with an existing lung or heart condition, or a weakened immune system, should contact a clinician early when respiratory symptoms change.
When another description is more accurate
Repeated throat clearing without a true cough can come from nasal drainage, dryness, reflux, or a throat sensation. Spitting out saliva is also different from phlegm produced by a chesty cough. A choking episode, sudden noisy breathing, or a feeling that something is stuck in the airway requires a different response, particularly if breathing or speech is affected. Describe exactly what you feel and what comes out rather than forcing it into either term.
How a clinician works through the cause
A clinician may ask when the symptom began, whether you have been around smoke or sick contacts, and whether you have asthma, reflux, allergies, or other lung conditions. They will consider breathing rate, oxygen level, chest sounds, temperature, and the character of the phlegm alongside your medical history. A photo of unusual sputum, a list of medicines, and notes about triggers can make the consultation more informative.
