Colposcopy vs HPV Test: What Is the Difference?

An HPV test detects high-risk HPV in a sample; colposcopy looks directly at the cervix when closer assessment is needed.

These are not interchangeable procedures. HPV testing is a laboratory test performed on a cervical sample. Colposcopy is an examination of the cervix using magnification, often after a screening result or visible concern needs investigation. A clinician may use information from an HPV test to decide whether colposcopy is appropriate, but the two tests provide different kinds of information.

Colposcopy may be recommended when an earlier cervical result needs closer assessment, when HPV-related findings persist, or when the cervix looks unusual on examination. During the procedure, a speculum is used and solutions may highlight areas for inspection. If a biopsy is advised, the sample helps determine what follow-up is suitable.

An HPV test may be useful when the clinician needs to check for high-risk HPV in a cervical sample. It can help classify a screening result or guide follow-up, depending on the screening pathway. A detected result is not a diagnosis of cancer, and an undetected result does not explain symptoms that need a separate clinical assessment.

HPV testing uses a cervical sample and may cause pressure, discomfort, or light spotting. Colposcopy can feel uncomfortable, and biopsy can cause short-lived cramping or bleeding. Use the clinic's aftercare advice if tissue was taken. Contact a healthcare professional if bleeding is heavy, pain is increasing, or fever or foul-smelling discharge develops.

Neither test is designed to investigate every pelvic or vaginal symptom. Bleeding after sex, bleeding between periods, unusual discharge, pelvic pain, or bleeding after menopause warrants medical review regardless of HPV status. A clinician may arrange an examination, infection testing, pregnancy testing, imaging, or another cervical investigation instead of—or alongside—these tests.

The recommendation depends on the exact HPV result, any cervical cell changes, previous screening and treatment, examination findings, symptoms, pregnancy, and local follow-up guidance. HPV testing supplies laboratory information; colposcopy supplies a direct view and can provide tissue for analysis. Ask what prompted the recommendation, whether biopsy is possible, how to prepare, and how results will be communicated.
