Why does ovarian endometrioma cause painful periods?

Short answer

Painful Periods is a common presentation of Ovarian Endometrioma because trapped endometriosis-like tissue reacts to menstrual hormones, causing inflammation and pressure in the ovary.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Main mechanism
Hormone-linked inflammation, blood-filled cyst pressure, and pelvic irritation
Symptoms to track
Timing, location, bleeding, bowel symptoms, urinary symptoms, and pain severity

What is happening in the ovary

An ovarian endometrioma is a cyst containing old blood and tissue linked with endometriosis. Around a period, hormone-responsive tissue can become inflamed. Swelling stretches the ovarian surface, while irritation may affect nearby pelvic tissue. These changes can produce cramping, deep pelvic aching, or pain that radiates into the lower back. Scar tissue can also limit how freely the ovary and surrounding organs move.

Pain may build before bleeding starts, become stronger during the period, or continue after the flow has ended. Severity can vary with the cyst's position, associated endometriosis, and individual sensitivity. Painful periods have other possible causes, so the symptom alone cannot identify an endometrioma.

Steps that can help

Record when pain begins, where it is felt, how long it lasts, and whether bleeding, bowel movements, sex, or urination make it worse. A clinician may assess your history, examine you, and use pelvic imaging to look for an endometrioma or other cause. Treatment may include pain relief, hormone-based treatment, monitoring, or surgery depending on symptoms, cyst features, fertility plans, and examination findings.

Use medicines only as directed and tell the clinician about pregnancy possibility, stomach problems, kidney disease, or other medicines. Heat over the lower abdomen and gentle activity may ease cramping for some people, but should not delay assessment when pain is new or severe.

When to seek medical care

Arrange medical review for period pain that is worsening, interrupts daily activities, returns despite treatment, or occurs with difficulty becoming pregnant. Seek urgent care for sudden severe pelvic pain, fainting, marked weakness, fever, repeated vomiting, or a swollen tender abdomen. These symptoms can signal a complication or another urgent pelvic condition.

Questions for your doctor

Ask whether imaging suggests an ovarian endometrioma, what other causes of painful periods should be considered, and how treatment may affect fertility. You can also ask which pain medicines fit your health history, what symptoms should prompt urgent help, and when follow-up imaging or review is needed.

Questions people ask

Can an ovarian endometrioma cause pain outside a period?

Yes. Inflammation, scar tissue, or pressure from the cyst can cause pelvic discomfort between periods as well as menstrual pain.

Does painful menstrual bleeding prove there is an endometrioma?

No. Painful periods have several possible causes. Assessment and, when appropriate, pelvic imaging help clarify the reason.
