Why does meningitis cause neck stiffness?

The meninges are thin layers that cover the brain and spinal cord. When they become inflamed, bending the head forward can pull on sensitive structures around the upper spine. Muscles may tighten defensively, and movement can feel painful or restricted. Neck stiffness is a very common presentation of Meningitis, but it can also occur with muscle strain, migraine, or other illnesses.

Do not try to force the neck to bend or massage a painful, stiff neck when you feel unwell. Rest in a comfortable position, avoid driving if you are dizzy or confused, and arrange urgent medical assessment when neck stiffness occurs with fever, a severe headache, vomiting, unusual sleepiness, or a new rash. A clinician may examine the nervous system and decide whether tests or treatment are needed.

Seek emergency care immediately for neck stiffness with confusion, difficulty staying awake, a seizure, breathing trouble, a rapidly worsening headache, or a rash that does not fade under pressure. Babies and young children may show irritability, poor feeding, unusual limpness, or a bulging soft spot instead of describing neck pain. Symptoms can worsen quickly, so do not wait to see whether they settle.
