What vomiting can mean during a migraine

Migraine can activate brain pathways that affect nausea and the stomach, so vomiting may accompany head pain, light sensitivity, sound sensitivity, or an aura. The main immediate concern is loss of fluid and salts. Dehydration can make headache, dizziness, tiredness, and nausea harder to settle. Vomiting that is new for you, much more intense than usual, or paired with a different headache pattern should not automatically be attributed to migraine. Other illnesses can also cause vomiting, and an assessment may be needed to separate them.

What to do at home

Rest in a quiet, dim room and take small, frequent sips of water or an oral rehydration drink. Large amounts at once may trigger more vomiting. When fluids stay down, try bland food in small portions. Follow the migraine treatment plan prescribed for you; if you cannot keep an oral medicine down, ask a clinician or pharmacist whether another formulation is suitable. Avoid driving while dizzy, weak, or affected by visual symptoms, and record the timing of vomiting, fluids kept down, headache features, and any medicines taken.

When to seek medical care

Call emergency services for a sudden, extreme headache; fainting; new confusion; trouble speaking; one-sided weakness or numbness; a seizure; a stiff neck with fever; severe drowsiness; or vomiting blood or material that looks like coffee grounds. Seek same-day medical advice if vomiting continues, you cannot keep fluids down, you are passing very little urine, your mouth is markedly dry, or standing causes significant dizziness. Arrange a routine review if vomiting repeatedly accompanies headaches, your usual migraine pattern changes, or treatment is no longer controlling attacks.
