When should you worry about testicular pain with an inguinal hernia?

An inguinal hernia occurs when tissue pushes through a weak area in the groin. Discomfort may travel towards the scrotum, particularly with coughing, lifting or standing. A bulge may reduce when lying down, but changes in pain or its ability to move back are more significant than appearance alone.

Testicular pain can also arise from infection, injury or twisting of the testicle. A clinician should examine both the groin and scrotum rather than attributing every ache to the known hernia.

Arrange a clinical review for new, persistent or increasing pain. Avoid heavy lifting and straining until assessed, and support the scrotum if that makes movement more comfortable. Do not force a painful bulge back into the groin.

Note whether the bulge changes with position, whether skin colour changes, and whether you have nausea, vomiting or bowel symptoms. These details help the clinician judge urgency.

Go to emergency care for sudden severe groin or testicular pain, a bulge that becomes firm or cannot be pushed back gently, skin turning red or dark, repeated vomiting, abdominal swelling, or inability to pass stool or gas. These can signal trapped or compromised hernia contents.

Seek urgent assessment as well for sudden testicular pain with nausea or an unusual testicle position, because torsion is a separate emergency. Do not wait for a routine hernia appointment.
