When to worry about pale skin with sickle cell disease

Sickle cell disease can affect red blood cells and oxygen delivery, so pallor may occur during anaemia or an illness. A person’s usual skin tone and baseline colour matter; compare with their normal appearance rather than relying on a single observation. Pallor can occur alongside fatigue, headache, dizziness, or yellowing of the eyes.

Contact the sickle cell care team or a clinician when pallor is new, persistent, or more noticeable than usual. Follow the person’s individual care plan, keep prescribed medicines available, and mention fever, reduced drinking, recent illness, or increased pain. Do not delay planned reviews or alter disease treatment without specialist advice.

Seek emergency help for severe breathing difficulty, chest pain, sudden weakness, confusion, fainting, a seizure, or pain that is severe or unlike the person’s usual episodes. Fever, extreme drowsiness, inability to drink, or a rapidly worsening appearance also needs urgent assessment. Tell the emergency team that the person has sickle cell disease.

Ask whether the pallor could reflect a change in haemoglobin, infection, dehydration, or another complication; which symptoms should trigger the emergency plan; and when blood tests or follow-up are needed. Confirm whom to contact outside clinic hours and which medicines should be continued during illness.
