When to worry about painful periods with an ovarian endometrioma

An ovarian endometrioma is a cyst associated with endometriosis. It may cause deep pelvic pain, painful periods, pain during sex or discomfort that is stronger on one side. Some people have few symptoms. Pain can also come from other ovarian or pelvic conditions, so a new symptom or a change in a known pattern needs clinical review rather than a self-diagnosis.

Write down the side and timing of pain, its relationship to bleeding, pain during sex, changes in bowel or bladder habits and any effect on daily life. Use only pain relief that is suitable for you, and ask a pharmacist if you are unsure. Follow the monitoring plan for the cyst and attend recommended scans or appointments. Tell your clinician about pregnancy plans, fertility concerns and previous ovarian surgery.

Arrange review if period pain is increasing, lasting longer, occurring outside menstruation or not responding to your treatment plan. Seek urgent care for sudden severe one-sided pain, faintness, marked dizziness, fever, repeated vomiting or a swollen abdomen. These symptoms can occur with complications such as twisting or rupture and need prompt assessment. If pregnancy is possible, pelvic pain or bleeding should be treated as urgent until a clinician has checked it.

Ask about the cyst’s size and appearance, the recommended follow-up plan, symptoms of twisting or rupture, fertility considerations and when surgery or another treatment might be discussed.
