When to worry about cough with asthma

Cough is a very common presentation of Asthma. It may reflect narrowed, irritated airways, especially at night, after exercise, or around triggers such as smoke, dust, cold air, or respiratory infections. A new pattern can indicate that asthma control has changed, even when wheezing is absent. Cough with fever, sharp chest pain, or feeling generally unwell may have another cause and deserves assessment.

Use your asthma medicines exactly as prescribed and follow your written action plan if you have one. Keep track of when the cough appears, possible triggers, nighttime waking, and how often you need your reliever. Move away from smoke or other known irritants, rest, and drink fluids if your throat is dry. Do not borrow someone else’s inhaler or change a preventer dose without clinical advice.

Call emergency services if breathing is very difficult, the reliever is not helping as directed, you cannot complete sentences, you become drowsy or confused, or your lips look blue or grey. Arrange same-day medical advice for a rapidly worsening cough, repeated nighttime symptoms, chest tightness, or a reliever that is needed more often than your plan allows. Book a review for a cough that persists, keeps returning, or limits activity.

Ask whether your inhaler technique is effective, whether your action plan needs updating, and what specific changes should trigger urgent care. Mention recent infections, workplace or home exposures, sleep disruption, and any difference in exercise tolerance.
