When to worry about buttock pain with gluteal tendinopathy

Short answer
Arrange medical review for buttock pain that is severe, worsening, unexplained, or accompanied by weakness, fever, or injury.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance

How gluteal tendinopathy feels
Buttock Pain is a common presentation of Gluteal Tendinopathy. The gluteal tendons connect the buttock muscles to the hip and help stabilise the pelvis during walking, stairs, and standing on one leg. Pain may sit on the outer buttock, worsen when lying on that side, or flare after hills, stairs, or a sudden increase in exercise. Burning pain, marked numbness, or symptoms travelling below the knee may point to nerve involvement rather than tendon irritation alone.
Practical next steps
Reduce the activity that clearly provokes pain without becoming completely inactive. Keep walking within a tolerable range, avoid prolonged hip compression such as lying on the painful side, and use a clinician-guided strengthening plan. A physiotherapist can check hip movement, tendon loading, walking pattern, and nearby sources of pain.
Warning signs
Seek urgent care after a significant fall if you cannot bear weight, or if the hip looks deformed. Get prompt medical advice for fever, a hot or swollen area, rapidly increasing pain, unexplained weight loss, new leg weakness, or loss of bladder or bowel control. These signs are not typical tendon-load symptoms and need a different assessment.
Questions to ask
Ask which movements are safe, whether the pain pattern fits gluteal tendinopathy, and how to progress strengthening. Ask how nerve symptoms, hip-joint problems, or a recent injury would change the plan.
