When should you see a doctor about kneecap pain?

Short answer
See a doctor about kneecap pain when it follows an injury, keeps returning, limits stairs or walking, causes swelling, or comes with instability or a visible change in position.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What pain around the kneecap can reflect
Front-of-knee pain may come from the joint surface, the tendon below the kneecap, nearby soft tissues, or how the kneecap moves during bending. Patellofemoral pain syndrome often hurts with stairs, squats, or prolonged sitting. Patellar tendinitis commonly relates to loading the tendon. Chondromalacia patellae involves irritation affecting the cartilage beneath the kneecap. In growing children and teenagers, Osgood-Schlatter Disease can cause tenderness at the bump below it.
A kneecap that shifts or dislocates needs assessment, especially when pain began suddenly or the knee looks different. The symptom location is useful, but it does not establish a diagnosis by itself.
Protecting the knee before review
Temporarily reduce deep squats, kneeling, jumping, and other movements that reproduce the pain. Keep gentle range-of-motion activity within comfort and use a cold pack wrapped in cloth if the area is irritated after activity. Wear supportive footwear and avoid repeatedly pressing the sore spot to test it. A clinician may consider exercise therapy or custom orthotics when foot and leg mechanics contribute.
When care is urgent
Seek urgent care if the kneecap appears out of place, pain follows a forceful blow or twist, the knee becomes rapidly swollen, or you cannot bear weight safely. Fever with a hot, red knee also needs urgent attention. Arrange a routine appointment for pain that persists, returns with activity, or interferes with sleep, school, work, or ordinary walking.
Questions for your appointment
Ask whether the pain comes from the kneecap joint, tendon, or surrounding tissue, and whether you need imaging. Ask which exercises and activities are appropriate, whether footwear or orthotics could help, and what change should prompt earlier review.

Questions people ask
Is kneecap pain common after increasing exercise?
It can occur when activity loads the kneecap or tendon faster than the tissues can adapt. Persistent or worsening pain still deserves an assessment.
Can foot alignment affect kneecap pain?
Foot and leg mechanics can influence how force travels through the knee. A clinician can decide whether exercises or orthotics are appropriate.
