When is topical retinoids recommended for acanthosis nigricans?

Acanthosis nigricans causes darker, thicker, velvety skin, often in folds such as the neck, underarms, groin, or knuckles. It can be associated with insulin resistance, weight-related metabolic changes, certain medicines, or other health conditions. The skin appearance alone does not identify the cause.

A topical retinoid can help loosen excess surface cells and improve texture over time, but it does not address insulin resistance or another underlying driver. Irritation is more likely in folds and on sensitive skin, so diagnosis and a gradual plan matter. Similar-looking rashes may need a different treatment.

Arrange a medical assessment for new or spreading dark, velvety patches, especially on the neck or body folds. Tell the clinician about medicines, menstrual or hormonal changes, family history, and symptoms such as increased thirst or urination. Use the prescribed retinoid sparingly and avoid applying it to cracked, sunburned, or inflamed skin.

Choose gentle cleansing and moisturising, and avoid scrubbing or bleaching products that can worsen irritation and darkening. Use sun protection on exposed areas. Do not use a retinoid during pregnancy unless your clinician gives specific advice; discuss breastfeeding and sensitive-skin concerns before starting.

Seek timely medical review if the change appears quickly, affects many areas, involves the mouth or palms, or comes with unexplained weight change or other new symptoms. Stop the product and contact a clinician for severe burning, swelling, blistering, or persistent dermatitis.
