Wheezing cough sound: what should you know?

Wheezing is commonly a whistle, squeak or musical tone, often clearer when breathing out. A barking, wet or harsh cough sounds different, so recording the noise can help a clinician understand what you noticed. Asthma presents with wheezing and cough that may change with exercise, cold air, allergens or night-time. Acute Bronchitis can make breathing noisy when inflamed tubes contain mucus. COPD can cause persistent wheeze and cough, while Bronchiolitis may cause noisy breathing in young children. Allergic Rhinitis can add throat clearing from nasal drainage.

If safe, note whether the sound occurs at rest, during activity, on breathing in or out, and whether it disappears after coughing. Record associated breathlessness, fever, mucus, chest discomfort, rash or exposure to smoke and allergens. Rest and avoid airway irritants. Use only medicines prescribed for you and follow their instructions. A clinician may listen to the chest, assess breathing and arrange tests. Nebuliser Treatment may be used in a monitored setting when inhaled medicine is needed for narrowed airways.

Seek emergency help for severe breathing difficulty, blue or grey lips, confusion, collapse, inability to speak comfortably or swelling in the throat, lips or tongue. A sudden wheeze after eating, taking medicine or being stung can occur with Anaphylaxis. Prompt assessment is also needed after choking, with chest pain, coughing blood, a concerning fever or rapidly worsening symptoms. For children, urgent signs include ribs pulling in with breaths, pauses in breathing, unusual drowsiness or inability to drink.
