Skin may still be adjusting early in a course, and inflamed spots can take time to settle. Ongoing lesions may also reflect missed doses, difficulty taking capsules as directed, an interaction with another medicine, or a condition that resembles acne. A flare, dryness, or irritation can make the skin look worse even when some acne activity is changing.

Keep a simple record of doses, new lesions, irritation, and other medicines. Do not increase, reduce, or stop isotretinoin without the prescriber. Use the recommended gentle cleanser and moisturiser, avoid picking, and tell the clinic about severe dryness or mood changes. The dermatologist may reassess the dose, schedule, skin-care routine, or whether another diagnosis needs attention.

Seek urgent medical advice for severe headache with vision changes, marked abdominal pain, yellowing of the skin, an allergic reaction, or thoughts of self-harm. Contact the prescribing team soon if acne becomes rapidly more painful, develops extensive crusting, or side effects prevent you from taking the medicine.

Ask whether the response is on track, whether the dose or schedule needs review, how to distinguish acne from another rash, and which products or medicines should be avoided during treatment.
