What if inhaler therapy does not relieve wheezing?

Wheezing is a musical or whistling sound caused by narrowed or irritated airways. It may continue if the inhaler technique is not delivering enough medicine, if a trigger is still present, or if the medicine does not address the cause. Viral illness, smoke, dust, allergy, reflux, and other lung or heart problems can also contribute. Describe whether the sound is heard while breathing in, breathing out, or both, and whether it is new or changing.

Move away from smoke, strong scents, dust, or another known trigger and sit upright. Use only the inhaler and dose in your personal plan. Ask a pharmacist or clinician to check each step of your technique and whether a spacer could help. Arrange a review if wheezing returns, lasts beyond the expected illness, disturbs sleep, or limits activity. Bring the inhaler, spacer, and a record of symptoms, triggers, and doses.

Get emergency help if wheezing comes with severe breathlessness, blue or grey lips, exhaustion, confusion, fainting, severe chest pain, or trouble speaking. Sudden wheezing with swelling of the lips, tongue, face, or throat may be a serious allergic reaction. Do not wait for repeated inhaler doses to work when these signs are present.

Ask whether the wheeze is coming from the airways and whether your inhaler is the right type for it. Ask for a technique demonstration, a clear action plan, advice about triggers, and specific thresholds for urgent or emergency care.
