What Happens If Spondylolisthesis Is Not Treated?

Spondylolisthesis is movement of one vertebra relative to another. Some cases remain stable, while others place ongoing stress on joints, discs or nearby nerves. This can cause lower back pain, tightness, difficulty with prolonged standing, or radiating leg pain. Nerve irritation may produce tingling, numbness or weakness. Sciatica is one possible pattern of leg symptoms, but symptoms can have other causes, so the diagnosis should be confirmed clinically.

Arrange an assessment when pain keeps returning, limits activity or is accompanied by leg symptoms. Physiotherapy may build trunk and hip support, improve movement habits and guide a gradual return to activity. Exercise therapy should be individualised rather than copied from a general routine. If symptoms remain disabling or nerve function changes, a spine specialist may discuss laminectomy or spinal fusion after reviewing examination and imaging findings.

Seek urgent care for rapidly increasing leg weakness, new difficulty walking, loss of bladder or bowel control, or numbness around the inner thighs or genitals. Arrange a prompt review for persistent radiating pain, progressive numbness or repeated falls. Sudden severe pain after an injury also warrants assessment.
