What happens if patellar tendinitis is not treated?

Short answer
Untreated patellar tendinitis may become persistent, making jumping, stairs, running and kneeling increasingly painful or difficult.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Often aggravated by
- Jumping, running, stairs or kneeling
- Rehabilitation focus
- Gradual tendon loading and leg strength
What persistent symptoms can mean
The patellar tendon connects the kneecap to the shin and transfers force when the knee straightens. Repeated loading without enough recovery can keep the tendon sensitive. You may shorten activities, lose leg strength, and compensate with the other leg. Knee pain or kneecap pain that remains after activity should not be dismissed as ordinary soreness.
A clinician can assess movement, training load and strength, while checking for other causes of front-of-knee pain. Symptoms can improve, but the pace differs between people.
A sensible treatment plan
Temporarily reduce the movements that sharply increase pain, then rebuild capacity gradually rather than stopping all movement. Physiotherapy and strength training rehabilitation can use controlled loading to improve tendon tolerance and leg control. Shockwave therapy or a PRP injection may be discussed in selected cases, but neither replaces a considered rehabilitation plan. Return to sport should be staged around symptoms and strength.
When to seek care
Arrange an assessment if pain lasts, worsens, limits work or sport, or returns whenever activity increases. Seek prompt care after a sudden snap, immediate swelling, a visible dip in the tendon, or inability to straighten the knee or bear weight, because these may indicate a more serious tendon injury.
Questions for your doctor
Ask which loads to modify, how pain should guide exercises, how strength will be measured, and when additional treatment or imaging is appropriate.

Questions people ask
Should I stop exercising completely?
Usually the painful load is modified while suitable movements continue; a clinician can set safe limits.
Can patellar tendinitis cause a tendon tear?
A sudden force can injure a tendon, so a snap, deformity or loss of knee extension needs prompt care.
