What does cough feel like with heart failure?

Cough is a common presentation of Heart Failure and may be dry, persistent, or worse when lying down. Some people notice coughing at night or after activity, together with breathlessness. A cough bringing up white or pink-tinged frothy mucus needs urgent assessment, particularly when breathing is difficult.

When the heart cannot move blood forward effectively, pressure can build in the blood vessels of the lungs. Fluid congestion can irritate the airways and make breathing harder. Cough may occur with swollen ankles, rapid weight change, tiredness, or needing extra pillows to sleep. These features can also have other causes, so a cough does not establish a diagnosis by itself.

If you have a known heart condition, take medicines exactly as prescribed and keep track of changes in breathlessness, swelling, and sleep position. Do not change a diuretic or restrict fluids unless your clinician has given you a plan. Rest with your upper body raised if lying flat worsens symptoms, and arrange a medical review for a new or changing cough.

Call emergency services for severe breathlessness, chest pressure, fainting, confusion, blue lips, or frothy pink mucus. Seek same-day medical advice for breathlessness at rest, rapidly increasing swelling, a new need to sleep upright, or a cough that is worsening with known heart disease. Bring a list of medicines to the assessment.

Ask whether your cough could reflect fluid congestion, whether your medicines need review, and which changes require urgent help. Mention whether it is dry or productive, when it appears, how lying down affects it, and whether swelling or reduced exercise tolerance has changed.
