What does ankle pain feel like with peroneal tendinopathy?

Short answer
Ankle Pain is a common presentation of Peroneal Tendinopathy. It is often a tender or burning ache along the outer ankle and foot that increases with walking, running, uneven ground, or turning the foot outward.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical location
- Behind the outer ankle and along the outer foot
- Common aggravators
- Running, uneven surfaces, and outward foot movement
Where the pain comes from
The peroneal tendons run behind the outer ankle bone and along the outside of the foot. Repeated loading can make this area sore to touch, especially after a change in training, prolonged walking, or frequent work on uneven surfaces. Pain may start during activity and linger afterward. Mild swelling or a snapping sensation can occur, but these symptoms overlap with sprains and other ankle problems.
Reducing tendon strain
Temporarily reduce the activity that reproduces the pain, while keeping gentle movement within a tolerable range. Supportive shoes and a gradual return to loading may help. A physiotherapist can assess foot and ankle control, then prescribe progressive calf and tendon strengthening. Avoid pushing through sharp pain or rapidly increasing distance, speed, or hills.
When to seek care
Arrange an assessment if outer-ankle pain persists, returns whenever activity resumes, or affects your walking. Seek urgent help after a sudden snap, marked swelling, inability to bear weight, deformity, numbness, or a foot that becomes cold or changes colour.
Questions for your doctor
Ask whether the tendons or another structure is involved, which exercises are suitable now, how to progress loading, and whether footwear or a brace would be useful.

Questions people ask
Is peroneal tendinopathy the same as an ankle sprain?
No. A sprain affects ligaments, while tendinopathy involves an irritated or overloaded tendon, although their pain locations can overlap.
Should I stop all exercise?
Usually, modify the provoking activity and keep comfortable movement; a clinician can guide safe tendon loading.
