What causes restless legs?

Short answer
Restless legs describes an uncomfortable urge to move, often worse while resting or at night. Possible contributors include iron deficiency anaemia, chronic kidney disease, peripheral neuropathy and some medicines. For some people, the pattern is restless legs syndrome without one clear underlying cause. Caffeine, alcohol, stress and disrupted sleep may make symptoms more noticeable, even when they are not the original cause.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How clinicians interpret it
Restless legs syndrome usually causes crawling, pulling, tingling or aching sensations that improve temporarily with movement and return during quiet periods. Peripheral neuropathy may instead cause ongoing burning or reduced sensation in the feet. Low iron can occur with tiredness, pale skin or breathlessness, while kidney disease may have few early symptoms. A medication review and blood tests can help separate these possibilities.
What to do next
Keep a short record of when symptoms start, what the sensation feels like, and whether walking helps. Protect sleep with a regular bedtime, gentle leg stretching and moderate daytime activity. Limit caffeine and alcohol near bedtime if they worsen symptoms. Do not start iron tablets without checking your iron status and the reason for deficiency; treatment should match the result and your medical history.
When to seek care
Book a clinical review if symptoms repeatedly disturb sleep, affect work or travel, or begin after a new medicine. Seek urgent help for one-sided leg swelling, sudden weakness, severe pain, chest pain or breathlessness, since these are not typical restless-legs symptoms. Mention pregnancy, kidney problems, heavy menstrual bleeding or a restricted diet because each can change the assessment.
Questions for your doctor
Could low iron, kidney function, nerve symptoms or a medicine explain this? Which tests are suitable? If iron is low, what is causing it and how should it be replaced?

Questions people ask
Is restless legs the same as leg cramps?
No. Restless legs is an urge to move with uncomfortable sensations; a cramp is a sudden, sustained tightening of a muscle.
Can iron tablets help?
They help when iron deficiency is present and treatment is appropriate, but testing and medical advice should come first.
