What are the alternatives to palliative care?

There is no single replacement for palliative care; alternatives or additional services may include cancer treatment, symptom-specific services, rehabilitation, counselling and hospice care.

What care may look like
Palliative care focuses on comfort, symptoms, emotional wellbeing and support for families at any stage of serious illness. It does not require stopping cancer treatment. If you do not want a specialist palliative-care team, your oncologist and primary doctor can still manage symptoms, while nurses, pharmacists, physiotherapists, dietitians, psychologists or social workers address specific needs. Cancer-directed treatment may shrink or slow a cancer and relieve symptoms; procedures can sometimes drain fluid around the lungs or abdomen. Hospice care focuses mainly on comfort when a person is nearing the end of life. The right mix depends on the cancer, symptoms, goals and available support.
Build a plan around your priorities
Tell the team what you want help with: pain, breathlessness, nausea, loss of appetite, sleep, mood, mobility, family strain or practical care at home. Ask which professional can handle each concern and how to reach them between appointments. If treatment is causing problems, ask whether the dose, schedule or supportive medicines can be adjusted. Care for lung cancer, pancreatic cancer, malignant ascites or malignant pleural effusion may involve different symptom services. You can accept some parts of supportive care and decline others.
When to seek urgent help
Seek urgent medical care for severe breathlessness, chest pain, new confusion, uncontrolled pain, repeated vomiting, inability to drink, heavy bleeding or sudden weakness. Contact the cancer team promptly if abdominal swelling, persistent abdominal pain, loss of appetite or reduced urine is worsening. Ask in advance who to call after hours so symptoms are assessed early.
