Wegener's granulomatosis and skin changes

This condition is an inflammatory disease of small and medium blood vessels. Reduced blood flow or inflammation in the skin can produce purple-red spots that do not fade when pressed, bruised-looking areas, painful ulcers, or firm nodules. Skin findings can appear with nasal crusting, sinus symptoms, cough, breathlessness, joint pain, fever, or marked tiredness.

A rash has many possible causes, so a skin change alone cannot establish this diagnosis. A clinician may examine the skin, nose, lungs, and blood pressure, then arrange blood and urine tests or specialist review. A chest X-ray may help assess lung involvement, while tuberculosis screening can be relevant when infection is part of the differential diagnosis.

Arrange a GP consultation for a new unexplained rash, ulcer, or purple spot, especially when it is painful, spreading, or accompanied by respiratory or urinary changes. Photograph evolving areas, list recent medicines and illnesses, and avoid applying steroid creams or taking leftover antibiotics without advice. Keep ulcerated skin clean and covered with a simple dressing.

Seek urgent help for coughing blood, severe breathlessness, chest pain, confusion, fainting, new weakness, very little urine, blood in the urine, rapidly spreading skin damage, or a severe infection-like illness. These symptoms can signal organ involvement or a serious alternative cause and should not wait for a routine appointment.

Ask whether the skin pattern needs a biopsy or blood tests, whether urine and kidney checks are needed, what lung symptoms to monitor, and whether a chest X-ray or specialist referral is appropriate.
