Use of Hydrotherapy: What to Know

Short answer

Hydrotherapy uses warm water, buoyancy, and guided movement to support rehabilitation while reducing load on painful joints.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Main tools
Buoyancy, warmth, and water resistance support guided movement.
Possible uses
Joint pain, stiffness, frozen shoulder, fibromyalgia, and ankylosing spondylitis may be treated with aquatic rehabilitation.
Do not enter
Avoid a shared pool with fever, contagious illness, or an uncovered wound.

How water changes exercise

Buoyancy supports part of the body’s weight, which can make walking, stretching, and strengthening more comfortable than land exercise for some people. Water resistance still challenges muscles, while warmth may help movement feel less restricted. A physiotherapist can adapt depth, speed, support, and exercise choice to your symptoms and goals.

Hydrotherapy may be included in rehabilitation for joint pain and stiffness, frozen shoulder, fibromyalgia, or ankylosing spondylitis. It is one part of a wider plan and does not replace assessment of the condition causing your symptoms.

Preparing for a session

Tell the therapist about pain changes, balance concerns, wounds, skin problems, continence issues, heart or breathing conditions, and any infection or fever before entering the pool. Wear suitable swimwear and use the pool’s handrails or assistance when needed. Begin with the prescribed intensity, pause if symptoms become sharp or unusual, and drink fluids after exercise. Progress should be guided by how your body responds, not by forcing a fixed routine.

When to pause and seek advice

Leave the pool and seek medical advice for chest pain, faintness, severe breathlessness, new weakness, or a sudden major increase in pain. Do not attend a shared pool with a fever, contagious illness, or an uncovered wound. Tell your therapist if joint swelling, dizziness, skin irritation, or fatigue repeatedly worsens after sessions so the plan can be adjusted.

Questions for your doctor

Ask whether hydrotherapy suits your diagnosis, what movements should be limited, and whether a hydrotherapy pool or land-based exercise therapy is more appropriate. Ask how progress will be measured and which symptoms mean you should stop a session.

Questions people ask

Is hydrotherapy only for people who cannot exercise on land?

No. It can be useful when water makes movement more comfortable or provides a different form of resistance, while land exercise may remain part of rehabilitation.

Can I start hydrotherapy during a painful flare?

Ask your clinician or physiotherapist first. They can check the cause of the flare and adapt or postpone exercise when needed.
