Short answer

A bursa is a small fluid-filled sac that reduces friction between moving tissues. Around the hip, irritation may affect the bursa over the outer hip, the iliopsoas region at the front, or the ischial area beneath the buttock. Pain location, tenderness, movement, and pressure help a clinician identify the likely pattern, although tendon and joint conditions can feel similar.

How the patterns differ

Lateral hip bursitis may hurt when lying on that side, climbing stairs, or walking for longer periods. Iliopsoas irritation can cause front-of-hip or groin discomfort with hip flexion. Ischial bursitis may be aggravated by prolonged sitting or pressure beneath the buttock. Visible swelling is not required, and Joint Swelling in another area may point toward a different problem.

Practical care

Reduce the positions and activities that repeatedly compress the sore area, but keep comfortable movement so the hip does not become guarded. A clinician or physiotherapist may address hip strength, pelvic control, gait, flexibility, and training load. Anti-Inflammatory Medication may be considered after reviewing your health history. If symptoms remain limiting, a Cortisone Injection or, in selected situations, Joint Aspiration may be discussed.

When to seek care

Seek prompt assessment for a hot, red, rapidly swollen area, fever, feeling acutely unwell, or inability to bear weight. Arrange review when pain follows a fall, causes night disturbance, or does not improve after activity changes. Sudden severe hip pain needs evaluation because infection, fracture, or another urgent cause can resemble bursitis.

Ask which bursa is suspected, what examination findings support that view, and whether the hip joint or nearby tendons also need checking. Discuss activity modification, strengthening, medication risks, and whether an injection or aspiration is appropriate.
