Two-level ACDF: what should you know?

ACDF means anterior cervical discectomy and fusion. Through the front of the neck, the surgeon removes one disc at each of two affected levels and takes pressure away from nearby nerves or the spinal cord when appropriate. Spacers are placed in the cleared spaces, and a plate and screws may help hold the neck steady while fusion develops.

The underlying problem may include cervical radiculopathy, which can send pain, tingling, or weakness into an arm, or cervical spondylosis, which describes age-related changes in the neck. The operation is planned around symptoms, examination findings, and imaging rather than the scan alone.

Before surgery, tell the team about medicines, allergies, swallowing problems, and any arm or walking changes. Follow the instructions about eating, medicines, wound care, and neck movement. Afterward, soreness in the throat or neck can make eating and speaking uncomfortable for a while; follow the prescribed diet and pain plan.

Walking is usually encouraged as directed, while heavy lifting, sudden neck movement, and driving should wait until the surgical team says they are safe. Keep follow-up appointments so healing, symptoms, and the hardware can be checked. A PEEK spinal cage may be one type of spacer discussed during planning.

Contact the surgical team promptly for worsening weakness, new loss of coordination, trouble breathing, inability to swallow fluids, fever with wound changes, or increasing neck swelling. Sudden severe neck pain after a fall or new loss of bladder or bowel control needs urgent evaluation.

Ask which two levels are involved, what symptoms the operation is intended to address, and whether a plate and screws or another stabilisation method is planned. Ask about activity limits, swallowing symptoms, follow-up imaging, and the signs of successful fusion.
