Understanding a child’s cough

Bronchitis-like illness often follows a viral infection and irritates the larger airways. Cough may linger after a child otherwise seems better. Wheezing, noisy breathing, fever, reduced appetite, or tiredness can occur, but breathing effort and hydration are more useful measures of severity than the sound of the cough alone.

A child health consultation allows the clinician to listen to the chest, check breathing rate and effort, assess hydration, and consider other conditions such as bronchiolitis. Antibiotics do not treat viral illness and should only be used when a clinician identifies a reason for them.

Home care and monitoring

Offer frequent fluids and continue breast or formula feeds for infants. Keep the child rested, clear the nose gently when needed, and use fever or pain medicine only according to age-appropriate medical advice. Avoid smoke, vaping, and strong fragrances around the child.

Watch how the child drinks, urinates, sleeps, and breathes. Do not give over-the-counter cough medicines to a young child unless a clinician or pharmacist confirms that the product is suitable. A nebuliser treatment is not automatically needed; it may be considered after an examination when a specific breathing problem is present.

When to get help

Get urgent help if the child is struggling to breathe, ribs pull in with each breath, pauses occur, lips look blue or grey, the child is unusually difficult to wake, or drinking is markedly reduced. Infants can worsen quickly, so trust a clear change from their usual behaviour.

Arrange medical review for persistent fever, worsening cough, wheezing, repeated vomiting, signs of dehydration, or symptoms that are not gradually improving. Mention the child’s age, temperature pattern, breathing changes, and fluid intake.
