What should you know about transplanted hair falling out?

The visible hair shaft can shed after transplantation while the follicle adjusts to its new position. Regrowth is gradual, so early shedding does not by itself show that the procedure has failed. The scalp may also be tender, swollen, crusted, or itchy for a period after surgery. Your clinician can judge progress by examining the recipient area and comparing photographs over time.

Hair loss can also affect non-transplanted hair. Ongoing androgenetic alopecia, female pattern hair loss, alopecia areata, telogen effluvium, traction alopecia, or scalp ringworm may cause shedding for different reasons. These conditions need different management, so do not assume every lost hair is from the transplant.

Follow the washing, sleeping, activity, and medicine instructions from your transplant team. Do not scratch, pick crusts, pull hairs, or apply unapproved products to the grafts. Keep scheduled reviews and document changes with photographs taken in similar lighting. Tell the clinician about recent illness, major stress, restrictive dieting, new medicines, tight hairstyles, or scalp treatments, as these details can help explain shedding. Finasteride treatment may be discussed for suitable people, but it requires individual medical advice.

Contact the transplant team promptly for worsening pain, spreading redness, increasing warmth, pus, fever, heavy bleeding, wound separation, or rapidly worsening swelling. Seek urgent care for trouble breathing, fainting, or severe facial swelling. Arrange a review if shedding continues, bald areas enlarge, or the scalp develops scaling, broken hairs, or painful patches.

Ask whether the timing and pattern fit expected shedding, how grafts and surrounding hair will be monitored, when regrowth should be reviewed, and whether another cause of hair loss needs testing.
