Tram tracking bronchiectasis

A bronchus can be seen from the side or along its length. When its walls are thickened and the airway is widened, the two walls may form a pair of lines. Radiologists call this tram tracking because the pattern resembles rails. It is a descriptive sign rather than a complete diagnosis, and the whole report matters more than this phrase alone.

The appearance may occur with bronchiectasis, where airways are structurally widened. People with this condition often report a long-standing cough or phlegm, although imaging and symptoms do not always match closely.

Ask the requesting clinician to explain which imaging test showed the sign, how extensive it is and whether comparison with an earlier scan is possible. They may assess breathing, listen to your chest, request spirometry or investigate repeated chest infections and other contributors.

Take prescribed medicines as directed and use any taught mucus-clearance technique consistently. Note the amount and colour of phlegm, fever, wheeze and changes in activity. Do not assume that a tram-tracking phrase means you need antibiotics; that decision depends on symptoms and examination.

Seek emergency care for severe breathlessness, blue or grey lips, new confusion, collapse or ongoing chest pain. Contact a clinician soon if you cough up blood, develop fever with worsening cough or phlegm, or notice a sudden decline in your ability to breathe or exercise.

Ask whether the tram tracking is limited to particular airways, whether it confirms bronchiectasis or needs another test, and how your lung function will be monitored. Ask for clear instructions about mucus clearance and the symptoms that should trigger urgent review.
