What should you know about total hysterectomy?

Short answer
A total hysterectomy removes the uterus and cervix, so periods and the ability to carry a pregnancy end; the ovaries may be left in place or removed separately.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What total hysterectomy means
The operation removes the womb and cervix. It does not automatically remove the ovaries or fallopian tubes, although your surgical plan may include one or both. If the ovaries remain, they can continue making hormones until natural menopause. The route may be abdominal, vaginal, laparoscopic, or a combination, depending on your anatomy, diagnosis, and the surgeon’s assessment.
A hysterectomy may be considered for problems such as fibroids, adenomyosis, endometriosis, heavy periods that have not improved with other care, or some womb or cervical cancers. It is a major decision because pregnancy will no longer be possible. Recovery and emotional responses vary, and pelvic symptoms may have more than one cause.
How to prepare and recover
Before surgery, review medicines, allergies, previous operations, fertility wishes, and whether the ovaries and tubes are planned for removal. Arrange help with meals, transport, children, and household tasks. Afterward, follow wound-care and activity instructions, walk gently as advised, and keep follow-up appointments. Avoid inserting anything into the vagina or lifting heavily until your surgical team says healing is adequate. Ask when driving, work, exercise, and sex can safely resume.
The reason for surgery affects what happens next. You can read about Uterine Fibroids, Adenomyosis, Endometriosis, Heavy Periods, or Pelvic Pain to understand related concerns.
When to seek urgent care
Contact your surgical team promptly for fever, worsening abdominal pain, foul-smelling discharge, heavy bleeding, a red or draining wound, persistent vomiting, or difficulty passing urine. Seek emergency help for sudden chest pain, severe breathlessness, fainting, or a painful swollen leg, because these can signal a serious complication.
Questions for your doctor
What exactly will be removed, and which surgical route do you recommend? What alternatives remain? Will my ovaries and tubes stay? How might this affect hormones, fertility, bladder or bowel symptoms, and sexual health? Which warning signs should prompt a call, and what is a realistic recovery plan for my work and daily activities?

Questions people ask
Does total hysterectomy remove the ovaries?
Not necessarily. Total hysterectomy refers to removal of the uterus and cervix; ovary removal is a separate part of the plan.
Will I have menopause immediately?
If both ovaries are removed, menopause symptoms can begin suddenly. If ovaries remain, hormone production may continue until natural menopause.
