Tight IT Band and Hip Pain: What to Know

Short answer
A tight IT band can sit alongside hip pain, but the source may be the outer hip, joint, back, or surrounding muscles.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance

- Outer hip, groin, buttock, or thigh
- Assessment focus
- Hip movement, strength, gait, and back symptoms
Understanding the symptoms
The iliotibial band runs from the outer hip toward the knee and helps transmit force as the leg moves. A pulling sensation on the outside of the thigh is different from deep groin pain, catching, or pain after a fall. Outer-hip pain may occur with gluteal tendinopathy, while groin symptoms can reflect hip impingement, a labral tear, or hip osteoarthritis. Sciatica can produce pain from the back through the buttock and leg. A hip fracture after an injury needs prompt assessment.
Practical care
Temporarily scale back hills, running, deep squats, or side-lying positions that reproduce pain. Keep gentle movement within a tolerable range and avoid repeatedly testing the painful motion. A clinician or physiotherapist can compare hip range, strength, gait, and the lower back. Treatment may focus on gradual gluteal and trunk strengthening rather than aggressive band stretching. Use a handrail on stairs if pain affects balance.
When to get help
Book an evaluation when symptoms persist, recur with ordinary walking, or cause night discomfort. Get urgent care after a fall if you cannot bear weight, or if the hip looks shortened or turned outward. Prompt assessment is also needed for fever, a hot swollen joint, progressive weakness, or new bladder or bowel problems.
Questions to ask
Where is the pain most likely coming from? Should I modify sleeping or exercise positions? Would testing for hip joint, tendon, or nerve involvement change treatment?

Questions people ask
Is every painful hip an IT band problem?
No. Hip pain can involve tendons, the joint, nerves, or an injury, so location and examination are important.
What should I avoid first?
Pause movements that reliably reproduce pain, such as steep hills or deep squats, while maintaining comfortable activity.
